  • Residential Case Manager +

    OVERVIEW:
    Under the administrative supervision of the Residential Site Director, who is the designated supervisor for Herrington Place, Poplar Bend and Camden Group Home, the Residential Case Manager is responsible for the delivery of case management services for a designated caseload of adult priority clients.

    MINIMUM QUALIFICATIONS:
    Must have a minimum of two years experience as a case manager and graduation from an accredited college or university with a minimum of a Bachelor’s degree in a health related field of counseling, psychology or social work. Individuals with a Bachelor’s degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen college-level semester hours of course work in behavioral health and an additional one year of work experience in the behavioral health setting.

    *The above qualifications express the minimum standards of education and experience for an applicant for this position. Other combinations of education and experience, if evaluated as equivalent, may qualify an applicant for consideration.

    DUTIES AND RESPONSIBILITIES:

    1. Provides case management services and completes related documentation in compliance with established criteria and time frames for such services

    2. Is involved in the initial intervention in crisis situations with case management clients and coordinates the provision of other needed crisis services 24 hour per day, seven days per week, and 365 days per year

    3. Is available as a member of a case manager team to case management clients 24 hours per day, seven days per week by means of an on-call cell phon

    4.Participates in weekly group supervision with a focus on client-specific and practicespecific activities

    5. Meets with a designated number of Residential Technicians on a rotating cycle in order to assess resident needs and address changes in treatment needs.

    6. Follows all guidelines outlined in the CAREY Policies and Procedures Manual including Corporate Compliance Policies

    7. Is expected to have regular and predictable attendance and the ability to work cooperatively with others.

  • Licensed Practicing Nurse, LPN +

    OVERVIEW:
    The Licensed Practicing Nurse is responsible for the provision of medical services in a community mental health center which provides outpatient, consultation, emergency, and residential treatment to residents of a geo-access area.

    MINIMUM QUALIFICATIONS:
    Licensure as a Licensed Practical Nurse in the State of Tennessee with all related credentials as a licensed practicing nurse.

    DUTIES AND RESPONSIBILITIES:

    1. Conducts examinations, provides direct treatment services, and reviews medical/psychiatric/psychological histories.

    2. Recommends referral to appropriate inpatient or to other treatment resources if beyond the scope of the Center’s services.

    3. Reviews treatment plans and signs treatment plans to initiate, continue or discontinue services.

    4. Consults actively with Center staff regarding treatment of clients in the Center’s care.

    5. Provides back-up consultations to Center On-Call staff providing emergency services.

    6. Maintains licensure and certification

    7.  Participates in quality assurance activities as evidenced by participation in the identification and resolution of problems identified by quality assurance activities, such as qualitative and quantitative peer review, direct observation, and other quality assurance monitors in place.

    8. Assists in the promulgation of medical/psychiatric policies and procedures, consults with the medical staff and designated administrative staff members regarding the quality of medical services.

    9. Follows all guidelines outlined in the CAREY Policies and Procedures Manual including Corporate Compliance Policies.
