  • Nurse Practitioner +

    OVERVIEW:

    The Nurse Practitioner is responsible for the provision of psychiatric evaluations and medication monitoring under the supervision of the Medical Director.

    MINIMUM QUALIFICATIONS:

    Licensed as a registered nurse and a nurse practitioner in the State of Tennessee with all related credentials as a nurse practitioner.

    DUTIES AND RESPONSIBILITIES:

    • Conducts psychiatric evaluations and follow-up medication monitoring and reviews medical/psychiatric/psychological histories.

    • Prescribes medications in compliance with established prescribing protocols.

    • Makes diagnoses in the appropriate terminology set forth in the Diagnostic and Statistical Manual when appropriate.

    • Recommends referral to appropriate inpatient or to other treatment resources if additional services are needed by clients that are beyond the scope of the Center’s services.

    • Reviews and signs agreement with treatment plans to initiate, continue or discontinue services.

    • Consults actively with Center staff regarding treatment of clients in the Center’s care.

    • Provides back-up consultations to Center On-Call staff providing emergency services.

    • Maintains licensure, D.E.A. certification and other qualifying credentials necessary to provide services under Federal and State guidelines.

    • Participates in quality assurance activities as evidenced by participation in the identification and resolution of problems identified by quality assurance activities, such as qualitative and quantitative peer review, direct observation, and other quality assurance monitors in place.

    • Assists in the promulgation of medical/psychiatric policies and procedures, consults with the medical staff and designated administrative staff members regarding the quality of medical services.

    • Follows all guidelines outlined in the CAREY Policies and Procedures Manual including Corporate Compliance Policies.

    • Is expected to have regular and predictable attendance and the ability to work cooperatively with others.

  • Residential Technician +

    OVERVIEW:
    Provides supervisory, monitoring, and basic care functions for residents of Carey Counseling Center Inc.’s Enhanced Supported Housing and Supported Housing programs. The Residential Technician is required to work in a cooperative effort with other residential staff to provide a safe, structured environment which promotes and facilitates overall health and wellness for residents with a continual focus on progress towards greater independence and less restrictive levels of care.

    MINIMUM QUALIFICATIONS:
    High school diploma or equivalent.

    DUTIES AND RESPONSIBILITIES:
    • Supervises and monitors resident’s compliance with house rules and behavioral expectations after consultation with Case Managers, Residential Program Directors, and Site Directors.

    • May assist in the purchasing of foods and cleaning supplies under budgetary guidelines for the residential program.

    • Follows weekly menus according to state and federal guidelines

    • Prepares meals according to established menus and/or monitors meal preparation by residents and compliance of residents with dietary guidelines.

    • Monitors residents’ compliance with expectations related to personal hygiene and appearance.

    • Completes household repairs when possible and if not, informs residential supervisory staff of any needed household repairs.

    • May be required to provide transportation for residents related to recreational, shopping, medical and other needs after consultation with residential supervisors.

    • Documents client progress on a daily basis.

    • Works with residential clients towards achievement of psychosocial goals through groups and activities as well as individual meetings.

    • Attends Residential staff meetings as scheduled by Residential Directors to discuss programmatic and resident-related issues.

    • Conducts facility inspections as needed to monitor cleanliness and neatness of the entire facility and other issues related to TDMHSAS licensure guidelines.

    • Cleans and repairs residential facilities when possible to ensure compliance with TDMHSAS guidelines.

    • Reports all problems related to residents and resident care to the Residential Site Director.

    • Conducts orientation to program with new residents on day of admission including reviewing rules, behavioral expectations, and outline of the residential psychosocial program.

    • Participates in all required CAREY Residential Program staff meeting and training events.

    • Follows all guidelines outlined in the CAREY Policies and Procedures Manual including Corporate Compliance Policies.

  • Mobile Crisis Responder +

    OVERVIEW:

    Under the supervision of the Crisis Director provides on-site emergency screenings, evaluations, and consultation to individuals experiencing a mental health crisis.

    MINIMUM QUALIFICATIONS:

    Graduation from an accredited college or university with a Bachelor’s degree in psychology, counseling, social work, or related field is preferred. Experience in a mental health setting is preferred but not required.

    DUTIES AND RESPONSIBILITIES:

    • A willingness to work in the environments needed to care for the consumer in the most appropriate manner. This includes but is not limited to, CAREY outpatient offices, jails, emergency room, and schools.

    • A willingness to meet and work with personnel from other community agencies to explain crisis services and develop methods of coordinating services.

    • Provide prompt, courteous service to clients, their families, and referral sources.

    • Provide and document efforts to ensure an adequate resolution to crisis situations through the utilization of all available resources.

    • Provide active and documented follow-up to clients to help ensure adequate completion of the crisis situation within 12-24 hours following the assessment.

    • Conduct crisis assessments in accordance with agency timeframe standards.

    • Complete assessments and other documents in a professional manner and according to designated timeliness.

    • Participate in training, in-service, and professional meetings as specified by the supervisor.

    • Utilizes personal vehicle to provide services to consumers in the community.

  • Residential Case Manager +

    OVERVIEW:
    Under the administrative supervision of the Residential Site Director, who is the designated supervisor for Herrington Place, Poplar Bend and Camden Group Home, the Residential Case Manager is responsible for the delivery of case management services for a designated caseload of adult priority clients.

    MINIMUM QUALIFICATIONS:
    Must have a minimum of two years experience as a case manager and graduation from an accredited college or university with a minimum of a Bachelor’s degree in a health related field of counseling, psychology or social work. Individuals with a Bachelor’s degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen college-level semester hours of course work in behavioral health and an additional one year of work experience in the behavioral health setting.

    DUTIES AND RESPONSIBILITIES:

    2. Is involved in the initial intervention in crisis situations with case management clients and coordinates the provision of other needed crisis services 24 hour per day, seven days per week, and 365 days per year

    3. Is available as a member of a case manager team to case management clients 24 hours per day, seven days per week by means of an on-call cell phon

    4.Participates in weekly group supervision with a focus on client-specific and practicespecific activities

    5. Meets with a designated number of Residential Technicians on a rotating cycle in order to assess resident needs and address changes in treatment needs.

  • Licensed Practicing Nurse, LPN +

    OVERVIEW:
    The Licensed Practicing Nurse is responsible for the provision of medical services in a community mental health center which provides outpatient, consultation, emergency, and residential treatment to residents of a geo-access area.

  • Regional Housing Facilitator +

    OVERVIEW:

    Under the administrative supervision of the Fiscal Director, the Regional Housing Facilitator is responsible for facilitating the implementation of the housing grant activities and performing other duties related to the enhanced linkage between consumers and affordable, accessible housing.

    MINIMUM QUALIFICATIONS:

    Graduate from an accredited college or university with a Master’s degree preferred, but a Bachelor’s will be considered with appropriate experience. Experience in a mental health setting and grant knowledge preferred but not required.

    DUTIES AND RESPONSIBILITIES:

    1. Convenes or participates in collaborative efforts within our region that bring community stakeholders together to increase and enhance the housing stock and associated support services.

    2. Conducts assessments to identify current availability and unmet need for permanent housing and support service recipient in their assigned region.
     
    3. Aggressively seeks out opportunities and collaborates with potential funding entities to leverage and secure funding to develop and preserve permanent supportive housing options for service recipients in local communities.
     
    4. Addresses and combats housing discrimination for persons with mental illness and cooccurring disorders. 
     
    5. Delivers at least two (2) outreach presentations each year to educate traditional and nontraditional community partners, including but not limited to, group home operators, social service providers, landlords, apartment owners and housing developers in our region.
     
    6. Submits quarterly reports that describe their activities and successes to the State’s Director of Housing and Homeless Services, Division of Mental Health Services.
     
    7. Submits semi-annual updates of housing units and funding developed since the previous report by January 31 and by July 31 to the State’s Director of Housing and Homeless Services, Division of Mental Health Services by electronic means in a format prescribed by the State.
     
    8. Follows all guidelines outlined in the CAREY Policies and Procedures Manual including Corporate Compliance Policies.

  • Case Manager +

    OVERVIEW:

    Under the administrative supervision of the Site Director who is the designated Case Management Supervisor for that site, the Case Manager is responsible for the delivery of case management services for a designated caseload of child and adolescent or adult priority clients. The Case Manager is under the clinical supervision of the Clinical Director or designee.

    MINIMUM QUALIFICATIONS:

    Graduation from an accredited college or university with a minimum of a Bachelors degree in a health related field of counseling, psychology or social work, or must be licensed as a nurse (LPN or RN). LPN’s must have fifteen semester hours of behavioral health and one year of behavioral health care experience. Individuals with a Bachelor’s degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen college-level semester hours of course work in behavioral health and at least one year of work experience in the behavioral health setting

    DUTIES AND RESPONSIBILITIES:

    1. Provides case management services and completes related documentation in compliance with established criteria and time frames for such services.  

           A. Provides home visits to clients, as needed, in order to link, monitor, and assess their overall functioning in their natural environment but in compliance, at a minimum, with the contact requirements of the level of the client’s case management.

           B. Is responsible for maintaining a designated amount of direct service time with clients and a designated amount of contacts for the assigned caseload.

           C. Participates in the initial individual program planning process with all service providers (i.e., therapist, medical staff) and provides ongoing information regarding client progress to the service providers.

           D. Provides assistance to clients in the utilization of community resources in the areas related to independent living skills (i.e., facilitating payment of rent, locating appropriate housing, or purchasing food).

           E. Provides transportation for clients, when no alternative transportation is available, to appointments for medical, counseling, and other essential services in order to facilitate the client's remaining on outpatient treatment status.

           F. Completes an individual program plan with each client within 30 days of enrollment into case management.

           G. Writes progress notes after each service contact with a case management client in accordance with the Center’s procedure on Time Frames for Clinical Records and Related Documentation.

           H. Maintains documentation of a six month service update in the clinical record.

           I. Completes a case management assessment prior to the development of the individual program plan.

           J. Participates in the development of a crisis/safety plan for priority consumers.

           K. Participates in regularly scheduled Peer Review.

    2. Is involved in the initial intervention in crisis situations with case management clients and coordinates the provision of other needed crisis services 24 hour per day, seven days per week, and 365 days per year.

    3. Is available as a member of a case management team to case management clients 24 hours per day, seven days per week by means of an on-call cell phone.

    4. Participates in weekly group supervision with a focus on client-specific and practicespecific activities on an individual basis with the case management supervisor until the supervisor determines that the case manager’s skill level warrants less intensive supervision.
